TRANSITIONS PROGRAM
Benefits to Transitional Care
¨ Help dealing with end of life issues. ¨ Familiar faces to assist in the transition from home health to hospice care. ¨ Provides continuity of care. ¨ Educating patients helps them make appropriate decisions to better utilize the services of home health and hospice care. ¨ Appropriate and timely services reduce patient/caregiver stress. ¨ Improved communication between health care providers, physicians, patients and their caregivers. ¨ Efficient planning minimizes end of life crisis decisions. ¨ Patients and families are not rushed to make decisions. ¨ Patient/caregiver support may be made available to assist the patient in choosing whether they want to use hospice or not. ¨ Bereavement support groups are available to those who have experienced a loss whether the patient transitions to hospice or remains in home care.

If If you answered "yes" to any of these questions, the Transitions Program may be of help to you.
Our Transitions Program Team: 1. 1.Nurse Case Manager Our nurse case manager can be an invaluable resource to individuals and families. They are available to visit your home to recommend equipment that you could use to make life easier, help you organize your medications, teach techniques for easier mobility, or talk about your illness and treatment options. 1. 2.Patient/Caregiver Support The Transitions Program provides patient/caregiver support to patients who are coping with a life threatening or serious illness. Our patient/caregiver support team is available to talk with patients and their families about fear and concerns about how illness will affect patients or their loved ones. 3.An Interfaith Minister We offer the services of a inter faith minister who is available when a person wishes to discuss his/her spiritual thoughts and concerns. 4.Trained and Dedicated Volunteers
Criteria:
Eligibility: Anyone diagnosed with a possible life limiting illness, that is not yet ready for hospice care.
Treatment: Patient is receiving aggressive or palliative care.
Duration: A Patient can be followed as needed or until a decision can be made whether the patient will stay in home care or transition to hospice care.
Transitions Program Case Manager:
Can assist the patient and caregivers by providing a list of services available to help meet the needs of the patient. Some services the case manager can assist with are: ¨ Contacting Meals on Wheels ¨ Arranging for Supportive Volunteers ¨ Counseling ¨ Arranging for Community transit to appointments ¨ Chaplain to discuss thoughts or concerns of a spiritual nature ¨ Arranging for Medical Equipment ¨ Medication Instruction ¨ Discussing Treatment Options
