Skilled Nursing Care …
available 24 hours a day, 7 days a week as needed
with round-the-clock care during periods of
medical crisis
Patient’s Primary Care Physician
… included as part of the hospice team,
overseeing patient’s care and providing
orders for needed drugs and medications
Medical Social Worker
… assists with emotional, social and family
issues and concerns, provides help with decision-making
and long-term care planning; accesses added community
resources
Certified Home Health
Aides … provide personal care,
light housekeeping, laundry, meal preparation
and occasional respite care
Spiritual Counselor
… ordained clergy member offering non-denominational
support for patients with personal or spiritual
concerns; acts as a liaison for patient’s
spiritual care advisor; available to provide in-home
religious or communion services; conduct liturgy
for the sick and dying; and available to officiate
at Funerals or Graveside Services at patient/family
request
Trained Volunteers
… provide companionship for patient and
respite for caregiver; able to assist with light
household chores, meals, and assist with bereavement
support groups and follow-up phone calls to family.
Can also assist with Crossings Hospice events
and fundraisers
