Durable Medical Equipment …
items needed to ensure patient safety and comfort
in the home, including hospital bed, over-the-bed
table, bedside commode, wheelchair—any
equipment related to the patient’s diagnosis.
Pharmacy Services
that specialize in providing drugs and in-home
IV therapy prescribed by the attending physician
to manage pain and disease symptoms; includes
Emergency Medical Kit which is safely and securely
maintained in the home with drugs typically prescribed
by the physician for pain or uncontrolled disease
symptoms such as nausea.
Physical, occupational and speech therapy
when needed to enhance the patient’s comfort,
safety and general quality of life
Nutritional counseling by a
registered dietitian to assist with the patient’s
dietary needs
Respite Care
Up to 5 days inpatient care in an extended care
facility. Designed to allow the caregiver to go
on vacation, attend an out-of-town family function,
or to just take a break from day-to-day caregiving
demands.
Bereavement Follow-up
… individual and family counseling for a
year following the death (or as needs arise beyond
that time) via phone calls, bereavement mailings,
or a lending library of books, videos and publications
on grief and bereavement issues
Support Groups and Educational Programs
… available for family members or the community
at large; with educational sessions and workshops
on end-of-life care, grief and bereavement offered
to community or church groups, area clergy and
health and human service providers.
