Crossings Hospice of the VNA not only makes this wish reality but, through our team of Hospice professionals, also ensures that the individual receives skilled nursing care, up-to-date pain and symptom management, added personal care by home health aides, counseling and practical support extended to the family unit, plus added assistance from trained volunteers. Crossings Hospice care can be provided in the individual’s home or in an assisted living or extended care facility.
Eligibility … individual must have a life-limiting diagnosis for which they are no longer seeking curative or aggressive treatment. Diagnoses can include cancer; end stage diseases of the heart, lung, kidney or liver; end stage Diabetes or Stroke, AIDS, ALS, Parkinson’s, Multiple Sclerosis, Muscular Dystrophy, or Alzheimer’s and other end stage dementias.
Making a Referral …
can be made by patient, family or friend, but
admission to Crossings Hospice requires that
the patient’s primary care physician be
willing to certify the patient as having a life-limiting
prognosis.
Admission Process …
once the referral is made and the patient’s
primary physician certifies them as hospice
appropriate, the Crossings Hospice team can
make the initial visit within 24 to 48 hours
based on patient needs and the family’s
schedule.
