When faced with a terminal illness, over 90% of Americans surveyed say that they would prefer to live whatever time they have left as comfortably as possible, surrounded by the people, pets and things they cherish.

Centre Crossings Hospice not only makes this wish reality but, through our team of Hospice professionals, also ensures that the individual receives skilled nursing care, up-to-date pain and symptom management, added personal care by hospice aides, counseling and practical support extended to the family unit, plus added assistance from trained volunteers. Centre Crossings Hospice care can be provided in the individual’s home or in an assisted living or skilled nursing facility. For patients who choose to remain at home, respite care can be a temporary relief provided for the family when the caregiver role becomes overwhelming. Bereavement support is available prior to and following the death of a loved one up to one year.

Eligibility … individual must have a life-limiting diagnosis for which they are no longer seeking curative or aggressive treatment. Diagnoses can include cancer; end stage diseases of the heart, lung, kidney or liver; end stage Diabetes or Stroke, AIDS, ALS, Parkinson’s, Multiple Sclerosis, Muscular Dystrophy, or Alzheimer’s and other end stage dementias.

Making a Referral … can be made by patient, family or friend, but admission to Crossings Hospice requires that the patient’s primary care physician be willing to certify the patient as having a life-limiting prognosis.

Admission Process … once the referral is made and the patient’s primary physician certifies them as hospice appropriate, the Centre Crossings Hospice team can make the initial visit within hours based on patient needs and the family’s schedule.

Emphasis on:

  • using a team member approach to plan, coordinate and provide care in the home setting. The hospice team members include: our Hospice Director, Nurses, Aides, Medical Social Worker, Volunteers, Spiritual Counselor, Dietary Counselor as well as others.
  • creating a Care Plan in conjunction with the patient, family and patient’s primary care physician who remains an important part of the hospice care team
  • keeping the patient as comfortable as possible, and enhancing his/her quality of life
  • recognizing the family as the unit of care by including them in any decision-making process; and by supporting them physically and emotionally in their caregiving role
  • offering effective palliative care using multiple methods of pain and symptom control
  • making Centre Crossings Hospice support available 24 hours a day, seven days a week, including holidays.
